What is the Sequence32 (“Sequence32”, “we”, “us”, “our”) clear aligner system?

Sequence32’s clear aligner system is a series of clear aligners that straighten your teeth. Each clear aligner should be worn 2 weeks and will put pressure on certain teeth to create gentle dental movement. The aligners are BPA free and barely noticeable. In order for the treatment to give good results, the Sequence32 clear aligners should be worn 22 hours per day. They should only be removed to eat and brush your teeth and floss. The average length of a treatment is 5 months, depending on the number of aligners that your Assigned Independent Dental Professional (“AIDP”, “AIDPs”). will determine necessary. You should be informed of benefits, risks and inconveniences before starting your treatment. Sequence32’s clear aligner system is an elective procedure, and the main risk to not doing it is staying just as you are.

Case selection

 

Your AIDP will determine if you are a candidate for Sequence32’s clear aligner therapy. If your AIDP determines that your case is too complex or not appropriate for Sequence32’s clear aligner therapy, you should consult your local dentist to discuss treatment options. If your AIDP determines that you are a candidate (with your chief complaint in mind), then following the steps of your treatment and wearing your aligners full-time will give you great results.

Clear Aligner benefits

Barely there

Clear aligners are not noticeable because they are made of BPA-free clear plastic. Most people will not know you have them on and they are very comfortable and easy to wear.

Cleanliness

Since clear aligners can be removed, this allows you to eat normally and brush and floss your teeth perfectly.

 

 

Whitening benefits

The benefit of a whitening treatment is to have whiter teeth by removing stains.

 

Disclaimer

Sequence32 clear aligner system will work so much better if you have healthy teeth and gums. It is your responsibility to go see your dentist regularly before the treatment, during the treatment and after the treatment to do all necessary restorations and check-ups and cleanings. It is also your responsibility to keep your teeth clean and healthy by brushing 2-3 times daily and flossing once a day.

 

Aligner Risks and side effects

 

Saliva

You can notice that you have too much saliva and that your speech is off the first few days that you start wearing aligners, but it is temporary.

 

Pain

When you start wearing a new pair of aligners, you can have pain or discomfort for a few days, but it will go away shortly. You can take acetaminophen if needed. Also, the inside of your cheeks and lips can be sensitive from the rubbing of the aligners.

 

Allergy

It is possible to become allergic to your clear aligners. If this is the case, please discontinue use immediately and contact us so we can let your AIDP know.

 

Cavities

Cavities may occur during your treatment. It is very important to continue visiting your dentist every 6 months before, during and after your clear aligner treatment and get cleanings and check-ups done.  All cavities must be restored prior to your treatment. Any cavities left unrestored will increase in size with the use of clear aligners, especially if you eat of drink anything sweet while wearing them or if you don’t brush and floss before placing them back on. This can cause sensitivity and in extreme cases, pain and tooth fracture.

 

Gum or Periodontal disease

Periodontal disease may occur during your treatment. You are responsible to make sure you have healthy gums prior to and during your treatment. It is important to visit your dentist every 6 months prior to, during and after your treatment and get cleanings and check-ups done. If periodontal disease remains untreated during a clear aligner treatment, it can lead to worsening of gum disease and loss of teeth.

 

Root resorption

Any dental movement can cause root resorption and same goes with dental movement from clear aligner treatment. Clients who have had previous orthodontic treatment with braces will be more prone to root resorption. Root resorption can lead to dental mobility and loss of teeth.

If root resorption is diagnosed by your regular dentist, please stop treatment. If primary teeth are still present, clear aligner treatment can cause root resorption to those teeth and you may lose them.

 

Nerve trauma

Dental movement can be traumatic to teeth and some teeth can die and need a root canal treatment. If your dentist advises you that it has happened, please do necessary treatment or tooth loss can occur.

It is impossible to know in advance which teeth this can happen to but teeth that have previously been restored are more at risk.

 

TMJ issues

During aligner treatment, problems can occur in the temporo-mandibular joint, and can cause pain, headaches, earaches. The chances of this happening are higher with teeth grinding or clenching, past trauma or accident, arthritis, heredity or some medical conditions. Please see your dentist if you are experiencing TMJ issues.

 

Permanent retainer

If you have a permanent retainer, buttons or attachments, they will have to be removed before starting treatment with Sequence32’s clear aligners. Teeth cannot move if they are glued to a permanent retainer, therefore by accepting this consent form, you are agreeing to having it removed prior to treatment and at your own expense.

 

Piercings

Piercings in the oral cavity or on lip area are contra-indicated during clear aligner treatment. They can result in dental fracture and cause interruption of clear aligner treatment.

 

Impacted and supernumerary teeth

If you have impacted or supernumerary teeth, clear aligner treatment is not a good idea. Sometimes teeth can become impacted during aligner treatment, that means submerged by bone.

 

Supraeruption

A tooth that is not properly covered by aligners can migrate out of socket, leading to tooth decay, periodontal disease, hard time cleaning and eventually loss of tooth.

 

Permanent dental work

If you’ve had implants or bridges done, clear aligners are not an ideal treatment because they can not move implants and bridges. Also, any permanent dental work like veneers and crowns might need to be replaced after clear aligner treatment, at your own expense.

 

Partials or complete dentures

Partial or complete dentures will need to be replaced at your own cost after clear aligners treatment because they will not fit anymore in the end.

 

Gum swelling

Clear aligner treatments can cause swelling of the gums because they are irritated by aligners. This is normal. Clients can feel swelling, numbness, pain, sensitive jaw muscles and more. If the tissue attachment between the two front teeth gets very swollen, you might need to get a frenectomy, which is a little surgery to raise that attachment. This should be done by your own dentist.

 

Swallowing

Aligners can break and be swallowed and inhaled.

 

Black triangles

When teeth are moved in an adult mouth, the little pink triangles of gum tissue between the teeth sometimes does not regenerate in areas of teeth that are crowded and can result in little black triangles between some teeth.

 

Bite

If your bite feels off during your clear aligner treatment it is normal. You might need bite adjustment by your dentist at the end of your treatment.

 

Retainers

Retention at the end of your clear aligner treatment is forever. This means that your retainers will have to be worn at night, always. If retainers are not worn, teeth can slowly shift back to their original position.

 

Health issues

It is important to complete your medical questionnaire truthfully because certain health issues can really affect clear aligner treatment: hormonal issues, bone problems, blood disorders, taking certain medications such as bisphosphonates.

 

Results

The human body can be unpredictable. The same treatment on two different people will have different results. To get the best results, your commitment to wearing your clear aligners all the time is of utmost importance. Even then, not everyone’s teeth will move at a good pace and certain people can still need refinements at the end of their treatment. Another impression kit will be sent to you if that is the case. Treatment outcomes cannot be guaranteed but we will work with you until you are satisfied with the results.

 

Risks of whitening treatment

 

Gum irritation

It’s very important to remove any excess whitening product from gum tissue once the trays are inserted with whitening product. If this is not done properly, gum tissue can be irritated by the whitening product as well as the throat.

 

Sensitivity

Teeth can become sensitive and give sensation of shock waves when the whitening product is in direct contact with them. Teeth that have fractures or unsealed restorations can even be irritated to the point of needing a root canal.

 

Restorations on front teeth

Any filling on front teeth or crowns, bridges or implants on anterior teeth will not whiten during whitening treatment and will need to be replaced after whitening. The replacement fees are at your own expense.

 

Realistic expectations

It is important to have realistic expectations of your whitening treatment. Not all types of discoloration respond well to whitening. If stains are very dark or very deep, you might have an improvement but not perfect results. If you have gum recession or periodontal disease, the area of your root that is exposed will not whiten properly also. Fillings and restorations will not change color during whitening also. Over time, even if you are happy with your whitening result, teeth will stain again, and a whitening treatment needs to be refreshed every 6 months.

 

Agreeing to Arbitrate

I agree that any disputes or controversies for products and services offered by Sequence32 will be resolved in Arbitration under the Arbitration Rules of the ADR institute of Canada, Inc. I agree that all disputes with Sequence32 and or the AIDP to every case, including but not exclusively to medical malpractice disputes will be resolved in Arbitration, outside of court.  The arbitration will take place in English or in French in Montreal, Quebec. I agree that class arbitrations and class action lawsuits are not permitted. I agree to not participate in a class action.

 

Consent

 

I consent to use Sequence32 as a teledentistry platform so that a dentist certified in my Province will engage in telehealth. I understand that telehealth includes looking at my impressions, my pictures and my medical and dental questionnaires and establishing a diagnosis and a treatment plan for my clear aligner treatment between me and a dentist certified in my province who has engaged Sequence32 to provide non-clinical dental support. Consent is an ongoing process and I can withdraw my consent anytime.

 

I certify that:

My dentist has done my check-up and cleaning.

My dentist took x-rays of my teeth.

My dentist has done all the work that needed to be done in my mouth including if necessary dental fillings and crowns.

My dentist has confirmed that I have no shortening of roots.

My dentist has confirmed that I have no periodontal or gum disease after probing my gums.

My dentist has confirmed that I have no impacted teeth.

My dentist has performed an oral cancer screening and has confirmed that I do not have oral cancer.

I confirm that I have no pain in any of my teeth.

I confirm that I have no pain in my jaw.

I confirm that I have no loose teeth.

I confirm that I have left no dental issues unresolved and have no infection in my mouth.

I confirm that I go regularly to my dentist for check-ups and cleanings.

 

I confirm that I read and speak English and have understood and agreed with all that I have read and that Sequence32 has made no guarantee and promises to me. I understand that there are risks involved in this treatment and I understand those risks. I understand that more than one dentist can view my records for evaluation, diagnosis and treatment set-up. I understand that before my aligner treatment, I can discuss with a dentist licensed in my province who engaged Sequence32 to facilitate treatment. I understand that my AIDP and Sequence32 cannot guarantee a specific outcome to my treatment. I understand that my molar bite, class 2 or class 3 will not be corrected with this treatment. If I have a class 2 or class 3 occlusion, I will be informed to consult an orthodontist for a more complicated orthodontic treatment. I understand that I can only obtain treatment if my case is not complex and only anterior teeth need to be aligned. I understand that my teeth will be straighter at the end of my aligner treatment but may still be compromised.

I consent that Sequence32 share my personal and medical information with third parties and partners for the planning of clear aligner therapy and manufacturing of clear aligners in accordance with the laws.

I give permission to Sequence32 to use my pictures and my first name for marketing and educational purposes without financial compensation given voluntary acceptance. I release Sequence32 of any copyright, trademark or intellectual property claims by me or any third-party related to my participation in clear aligner therapy.
